ONC information blocking example from NPRM

In the ONC Cures Final Rule, they did not give too many examples of information blocking, but instead, they referenced the examples they shared in the NPRM. I've pulled those examples from the NPRM and pasted them in this PDF. They are useful in giving an illustration of the types of information blocking which can occur.
